A Family Model Provider opens their home and their heart by providing care to individuals with disabilities in a family home setting, as opposed to individuals living in an institutional setting. Family Model Providers provide housing, supervision, support and personal care to one or more individuals. Support may include assistance with daily living activities, such as bathing, personal hygiene, toileting, ambulation, dressing, eating, and meal preparation. Other supports include transporting and attending medical appointments, community outings and recreation, as well as medication administration which requires medication certification by the Family Model Provider, if the person cannot self-administer. The level of care required by the Family Model Provider varies according to the disability and needs of the individual. Mid-South Senior Care provides this service ranging from young adults to the elderly, and disabilities range from an intellectual disability, to physical disabilities. Mid-south Senior Care utilizes a personal matching system, considering the personalities, lifestyles, and care required to ensure the best placement possible. Family Model Providers and potential individuals for placement meet and interview one another and agree to services before any placements are made. Placement is desired to be long-term, as this is not a temporary foster care service.
Support and Compensation
Family Model Providers receive a generous non-taxable compensation, including a monthly room and board stipend. Compensation varies according to the funding source and service and level of need of the individual. This is a contractual position and the Family Provider is responsible for their own taxes. Mid-South Senior Care provides introductory and on-going training, and administrative support, direction, and advocacy. Housing modifications may be provided for individuals in wheelchairs or for other adaptations needed to support the individual.
Qualifications
Caregiving experience is required. Family Model Providers may be single or have family. Candidates should be age 21 or above. Rent or own home in Tennessee with home owners or rental insurance. Own or lease reliable vehicle. Have a valid TN Driver's license with auto insurance. Pass a drug screen, required background checks, and reference check. Provide or obtain TB screening. CPR & First Aid certification (or willing to obtain). Provide or pass other required trainings. Pass a home inspection. Ability to read and write in English and have basic computer skills.
