Nacogdoches Memorial Hospital’s Acute Rehabilitation Unit (ARU) seeks a Clinical Liaison to coordinate referrals, admissions, and outreach for inpatient rehab services. The Clinical Liaison conducts clinical screenings, educates patients and families on rehab options, and collaborates with physicians, payers, and the interdisciplinary team to ensure safe, timely transitions of care. This role builds and maintains relationships with community providers, supports program growth through marketing activities, and upholds high standards of clinical documentation, quality, and patient-centered service in a collaborative, team-based environment.
